Income limit varies by child's age and parents situation (ie: pregnant/disabled). Social Security number and income must be verified. After the application is electronically submitted, fax copies of documents: birth certificate, proof of any household income for the last 4 weeks, social security cards, IDs to fax number:866-658-2172. Write your ACCESS number on every page you fax and save your confirmation page.
