Eligibility Requirements and Important Information Pediatrics: Some of the pediatric services offered at the health department include: Comprehensive health care for both sick and well infants and children through age 17 School and sport physical exams Screening and diagnostic testing for hearing, vision and blood lead levels Measurement of growth and development Nutrition counseling Treatment or referral to specialty care Childhood immunizations Payment is based on a sliding fee scale. Appointments for existing patients can be made by calling our Pediatrics clinic at either (772) 794-7410 or (772) 794-7415. Adult Health Appointments: If you are currently a patient at the health department, appointments can be made by calling either of our Adult Health clinics at 794-7420 or our Women's Health clinic at 794-7425. If you are not currently a patient but would like to make an appointment, you will need to register as a new patient. Registration Information for New Patients When registering as a new patient, please bring the information listed below. If you want to see if you qualify for reduced medical fees on a sliding fee scale, visit the Eligibility page of the Health Department's website for more information. Drivers license or other ID listing home address, Social security card (including all dependent children in the family), Most recent paycheck stub, and Any insurance/Medicare/Medicaid cards Dental Health APPOINTMENTS: Appointments for established patients can be made by calling the dental clinic at 772-794-7435, Monday through Friday, 8am - noon and 1pm - 5pm. If you need to cancel an appointment, please notify the clinic of the cancellation at least 24 hours before to the scheduled appointment time. Appointments that are cancelled with less 24 hours notice will be considered a missed appointment. EMERGENCY WALK-IN SERVICES: Persons in need of emergency dental care can register for Dental walk-in services between 7:30am and 8:30am on TUESDAYS AND WEDNESDAYS only. Please call first to verify emergency walk-in days and times. For Medical services on a sliding fee scale, must be an IRC resident. Restricted eligibility for dental services. |
